Two schema faults that only appeared on deploy — the hand-applied column that was flagged and not applied, and a frozen ddl-auto CHECK on media_sessions that would have failed every thread creation invisibly behind it — plus the four places the implementation deliberately diverged from the design. A spec that quietly disagrees with the code is worse than no spec.
