Persist conversion_error instead of failing silently

Both the template and regular upload conversion pipelines swallowed Docs conversion failures (logged only), leaving is_converted=false with no recorded reason — so the doc manager showed a misleading "Document not converted" for every doc when Docs's convert-to was broken. Now the failure reason is written to ImgDat.conversion_error (truncated to 1024) and cleared on success, so failures are visible and surfaceable.
