Adds PER_MAILBOX pricing, the mail cost mode, and a stereotype-free aggregator: hosted bills every box on the server, third-party bills only what was mapped into Kamo. Deduped by address, so a mailbox six people share is one charge. Aliases are counted and never billed. An exhaustive switch with no default arm classifies every provider, so the next one added is a compile error rather than silently unbillable.
